There are three ways to make a referral to VSH:

1. Click here to print out the referral form then Fax the completed form to 919-854-1155 with the patient record.
   
   
2. Call 919-233-4911 and ask to speak to one of our doctors.
   
   
3. Click here and get the process rolling by filling out our electronic referral form.

Choose option number two, calling one of our doctors, if a patient needs to be seen within 24 hours of your referral.
