Emergency Pre-Registration Form

If you do not know the exact date of birth, can you tell us about how old your pet is?
If you do not know the exact date, do you know if your pet is current?
If yes, please list drug name, how much and how often your pet gets them.
If you do not know the name of the doctor, what is the name of the veterinary hospital?
Based on an initial assessment if your pet is in critical condition, medical care must begin immediately to prevent further deterioration and make your pet comfortable. Our doctors and emergency staff will first focus on working with your pet and then will contact you as information becomes available. We want you to be aware of the usual treatments and costs associated with emergency stabilization. This is only an estimate of what might be necessary immediately. An updated estimate will be provided by the doctor once they have performed an initial assessment (triage) and stabilization has begun. Initial treatment will include an examination by the doctor and may also include: intravenous catheter placement, intravenous fluid therapy, basic blood tests, medication(s) for stabilization (i.e. drugs for seizures or low blood sugar), clipping and cleaning of wounds, oxygen administration, and analgesia (pain relief). The cost for these treatments is $375-700.
